This form must be completed before riding the Lions Legacy shuttle.

Shuttle Authorization & Liability Waiver

I understand that transportation provided by Lions Legacy Community Development is not affiliated with any school district, city, or governmental entity, including Lockhart ISD.

By signing below, I agree to release Lions Legacy Community Development, its staff, volunteers, and affiliates from any and all liability in connection with participation in shuttle-related travel, including but not limited to injury, accident, or property damage.

I acknowledge that Lions Legacy will take reasonable precautions to ensure rider safety. I assume all risks associated with travel.

Media & Photo Release

I give permission for Lions Legacy Community Development to take photos or video of the participant during activities and shuttle use for documentation and promotional purposes. These images may be shared with current or future sponsors and supporters to highlight the impact of the program.

Shuttle Waiver

• I understand this shuttle is not affiliated with any school district or government entity. • I release Lions Legacy from liability for any injuries, accidents, or property loss. • I grant permission to be photographed for program • I am voluntarily participating in this shuttle program. • A designated Lions Legacy staff member will act as a responsible guardian for all youth participants during the shuttle trip.
